Healthcare providers are encouraged to report local cases to the Clark Health department. If the patient’s residence is unknown, reports should be directed to the health department in the area where the provider or laboratory is located.
When Township of Clark personnel are unavailable, providers can report cases directly to the New Jersey Department of Health (NJDOH) Communicable Disease Service (CDS) by calling 609-826-5964.
